Media uploader forms: get valuable insights from visitors

Collect more than just content—ask meaningful questions to better understand your audience.

The Media Uploader Forms feature allows you to add custom questions to your media uploader in a flow. This helps you gather additional insights from users alongside the UGC they submit.

Media Uploader Forms are a premium feature. To activate it, please contact your Customer Success Manager.

Creating a form

To create a new form that appears during the media upload process:

  1. Log in to your Flowbox account.

  2. Go to Visual Marketing in the left menu.

  3. Click on Media Uploader Forms.
    Click Create.

  4. Give your form a name.

Adding questions to your form

  1. From the Media Uploader Forms list, click on the form you want to edit.

  2. Select the language of the form.

    1. Languages are automatically shown to users based on their region. Click here to see all supported languages.

  3. Click Add a Question and enter your question.

  4. You can create up to 10 questions per form.

Enabling a form on your media uploader

To make a form available for users during media upload:

  1. From the Flowbox homepage, go to Visual Marketing in the left menu.

  2. Select the Flow where you want to enable the form.

  3. Click the Settings button on the right side.

  4. In the Flow settings page, go to Media Uploader.

  5. Scroll down to the Media Uploader Form section.

  6. Toggle Enable Media Uploader Form and select your desired form template from the list.

Collecting Content and Responses

Once a user uploads their media, they’ll be prompted to answer the custom questions you created in the “Experiences” step.

After submitting

  • The media is sent to the Inbox tab of the selected Flow.

  • All form responses will be visible in the Engage tab, right below the caption field of each post.
